Registered Nurse (RN) | Case Manager Location: Mission Hills, CA Agency: Revolution Healthcare Services Pay: $2,558 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/23/2026 About the Position Job Opportunity – Hospital Case Manager – California Start Date: February 2026 Location: Mission Hills, California Compensation: Details provided to qualified applicants Schedule: Full-time with weekend rotation to meet departmental needs; 1:20 patient ratio Highlights: Acute care facility setting with comprehensive care coordination and discharge planning responsibilities Patient ratio of 1:20 with no floating requirements outside the facility Epic charting system experience required 40 hours of orientation provided to support your transition Work with diverse patient populations including adolescents, adults, and geriatrics Interdisciplinary support including social services and transportation teams Requirements: Active California RN license (pending licenses accepted if active by start date) Minimum 2 years of acute care hospital case management experience in a hospital setting Strong background in care coordination and discharge planning within an acute care environment Proficiency with Epic charting system and computerized documentation Experience utilizing InterQual Criteria and DRG (Diagnosis Related Groups) Demonstrated competency in arranging DME, SNF, and ambulance transport independently Knowledge of HMOs, IPAs, and CMS regulations Proficient in Microsoft applications Ability to manage complex patient situations including homelessness, mental health crises, advance directives, and protective services processes First-time travelers considered with clinical screening Why It's a Great Opportunity: This position offers the chance to make a meaningful impact on patient outcomes through comprehensive care coordination in a supportive acute care environment.
