Job Description Overview: Our client, provides access to one of
the largest sources of contract clinicians, seeks an accomplished
Medical Social Worker (Acute Care Hospital) . Candidate must be
authorized to work in USA without requiring sponsorship Location:
Burbank, CA 91505 Duration: 13 weeks w/ possibility of extension
Notes : Schedule: Day Shift: 5x8 (8:00 AM – 4:30 PM). Weekend
Requirement: Typically, one full weekend every 4 weeks (subject to
department needs). Scrub Color: Business Attire or Scrubs. 100%
Onsite role. Description : The Medical Social Worker will provide
comprehensive social work services in a short-term acute care
inpatient hospital setting. This role supports adult and geriatric
patients through assessment, care coordination, and discharge
planning, working collaboratively with an interdisciplinary care
team. Key responsibilities include conducting psychosocial
assessments, developing, and implementing individualized plans of
care, coordinating safe and timely discharges, and participating in
care conferences and family meetings. The Social Worker will assist
patients and caregivers with complex needs including substance
abuse, homelessness, mental health crises, advance directives,
POLST, and protective services processes. The role requires strong
collaboration with physicians, nursing staff, case management, and
community resources to ensure continuity of care. Patient caseload
may be up to 25 patients, with a typical day-shift ratio of
approximately 1:25–35. Qualifications : Master’s degree in Social
Work (MSW) or LCSW required. Valid California Social Work license
required (pending license accepted if active by start date); LCSW
either/or MSW. Minimum of 2 years of acute medical social work
experience, including both social work and discharge planning
responsibilities. Knowledge of drug/alcohol abuse treatment
options, patients experiencing homelessness, patients with mental
health history or in crisis, advance directive, POLST, protective
services process, etc. Experience in short-term acute care
inpatient hospital settings. Proficiency with Epic electronic
medical records. Strong skills in assessment, care coordination,
utilization review, and discharge planning. Knowledge of community
resources, substance abuse treatment options, mental health
services, and social service systems. Ability to organize and
participate in family meetings and interdisciplinary care
conferences. Participate in the development of the treatment plan.
Strong communication, documentation, and organizational skills.
